Admissions Process
- There are 2 ways to apply: Online or by filling out a PDF form then emailing or faxing the completed application. If downloading PDF, fill out and fax to 815-324-1116 or email to cmather@willcountyillinois.com
- Contact Charlotte Mather – Admissions regarding bed availability at 815-774-4382.
- Confirm OBRA prescreen is complete and valid. Senior Services completes them for free to Will County Residents. Call 815-740-4225.
Admissions will contact you when there is a bed available. You will have 2 days to provide current medical and insurance information. You will then have 4 days to move in; otherwise the available bed will be offered to the next potential resident on the waiting list. You have the right to decline and maintain your spot on the waiting list.
*We do not accept anyone with a felony background or current smokers.
